Two Easy Ways to Refer Patients

Referring patients takes just a few minutes using our web referral or fax. Any type of healthcare professional (i.e., doctor, registered nurse, dentist, therapist, pharmacist, etc.) can make a referral. We’ll do the rest, contacting your patient within 24 hours. Patients do not need to have health insurance to be referred.

For professionals referring patients that have a medical condition that may interact with the use of nicotine replacement therapy, please use the fax referral form.
